Insurance Information
Like most practices offering wellness services including acupuncture and medical weight loss, we are currently out of network providers. This means that we have not signed onto medical insurance plans. The reason behind this is so our physicians can spend more time with you on every visit and so you can receive as much attention in relation to your medical problem as possible. Therefore, the patient will pay the full amount of the billable charge at the time of service. Remember we treat you as a person, instead of for example as patient #27 out of 40 for the day.
We will be more than happy to give you a copy of the receipt with the medical billing code(s) so you can submit it to your insurance carrier for possible reimbursement. Sometimes patients are also able to use network benefits such as health saving plans to help pay for our services. Also most of the radiology studies such as x-rays and blood work may be billed for your insurance carrier. Please note that since Dr Cornejo is an out of network provider, it is very important to contact your insurance carrier if your insurance covers a imaging study such as xray, MRI, CT scan, from an out of network provider. Also be sure to ask your insurance carrier how much if any you are responsible for.
For Orthopedic Medicine/Sports Medicine/Osteopathic Manipulation: As with the other services, the patient will pay the full amount of the billable charge at the time of service. Dr. Cornejo’s biller will submit the bill to the patient’s insurance company. Most of the time, the patient will receive a reimbursement check for the “out of network provider service.” We cannot guarantee how much if any of the visit fee will be covered. It varies from insurance carrier to insurance carrier. It should be noted that sometimes, although, rare, the patient may not be reimbursed and will be responsible for the full amount.
Medicare Patients: We do not participate in Medicare. Medicare patients must sign a billing waiver stating that the patient understands we do not participate in Medicare and that neither the physician nor the patient can file a claim to Medicare for the provided medical services.
